i had an interesting day yesterday 2 clients were commenting upon futher efforts. a new client who established care six months ago, presented yesterday with a pile of personal journals and animated discussion about behavioral therapy. they have been participating in for over three years. the client described DBT for their underlying BPD and moderate to severe depression w/ psychotic features. the dialectic behavioral therapy was explored further w/ our discussion because of my poor comprehension of the subject matter. according to the client who underwent 24 months DBT orientation comprising individual and group therapy and self study. The DBT provided a framework by which to structure their day thought and behavior and perhaps help this client from institutional treatment or from long term institutional custodial care.

the second client presented w/ chronic insomnia. difficult to treat either behaviorally, cognitivley or w/ non prescription and prescription medication. finally for the first time the client came for an appointment and promplty told me that they ’slept deeply’ the client dreamnt vividly, some violent images and some shameful. we began discussing how we are not responsible for our dream content but at this stage it may help to keep a personal diary. the discussion continued to include family. this discussion about the family member who was institutionalized w/ schizophrenia and the client is worried about these genetic traits.
